The larger "guru bead" marks the end of a cycle and is not counted.

Hold the mala in your dominant hand between the thumb and middle finger. Begin by touching the guru bead, the largest bead, then move through the beads one by one, counting your breaths (one breath is an inhale and an exhale), or reciting your mantra or prayer. When you reach the guru bead, reverse direction and repeat as many times as you'd like.
